Our House Calls Direct medical services are backed by the entire Care Team at MainStreet Clinic.
Gabi, Diedra, and Kelly provide back-office and clinical support for all House Call members and their families:
- Coordination with the staff at senior living locations
- Answering membership questions and onboarding new members
- Family support
- Specialist referrals and coordination
- Arranging for home health, wound care, durable medical equipment (DME), and other needs

House Calls Direct brings together a unique Care Team to provide the exact care program that each member needs:
- Expert care for all medical problems
- Medication management and regular, careful review of all your prescribed medications
- HIPAA-compliant communication with you, your family, your specialists, home health nurses and therapists
- Scheduled wellness visits and availability for urgent situations, also
- Option to come to MainStreet Clinic for office visits with Dr. Birdwell or Mary Anne Derrico, no additional charge
Following a comprehensive assessment, the care team leader and primary care physician assemble your Care Team that may consist of visiting nurse, respiratory or other technicians, social worker, or durable medical equipment provider.
We are geriatric care-focused, experienced with complex and chronic disease care, and committed to reducing unwanted visits to the ER or hospitalizations.
